Getting started is simple once you get past the all-Japanese menus. At the title screen, press Start to reach the main menu. The first option is likely "Exhibition Game" (usually the top choice), so highlight it and press X to confirm. You'll then pick a team from a list of NPB clubs - each is represented by a handful of Japanese characters and a team color. Use the D-Pad to scroll and X to select your team, then do the same for the opponent. After that, the game will show the lineup and you can start the match by pressing Enter or X.

The core loop is standard baseball: pitch using the D-Pad to aim and the A button to throw (with timing for different pitches), and bat by pressing X when the ball comes. Fielding is automatic for moving your player to the ball, but you need to press the B button to throw to a base (use the D-Pad to select which base). The game plays nine innings and the team with more runs wins. There's also a season mode, but navigating those menus requires some trial and error since everything is in Japanese.
